  1. Go to your local Persons with Disability Affairs Office (PDAO) and apply for a Person’s With Disability identification card for your psycho-social needs. Under Magna Carta for Disabled Persons, PWDs are entitled to have 20% discounts on purchase of medicines.

  2. Fill up an application form for PWD ID but be sure to secure a copy of your Medical Certificate and Clinical Abstract from your doctor stating the diagnosis and category of your disability, 3 pieces 1x1 ID picture, valid ID, and Barangay Clearance or Certificate.

  3. Photocopy all the required documents and submit it to your local PDAO and then follow any further instruction of the social worker on that day. And poof! You just have to wait for a while and your ID will be ready.


Mental health is as important as your physical health and it’s high time that we become more open about it. Depression and other mental illnesses may only be in your head, but that doesn’t mean it’s only make-believe. Point being, getting treated is not shameful after all.
